• Automobile Liability Policy 300/100 with $50,000 property damage
• Workers comp. insurance policy

Vehicle Registration

Certificate of Trade Name

• Getting a Certificate of Trade Name is straightforward. Simply visit your local town hall and request the ‘Certificate of Trade Name’ form. Fill it out, get it notarized, and submit it back to the town hall to obtain your official DBA (doing-business-as) trade name.

To start driving with us, you need to be an insured Independent Contractor with an active DBA status. Please download the W-9 form, and to get your insurance for your application. Email the documents to us at asmith@eliteprocuresolutions.com. We're here to help with any document-related inquiries.
